Minimum Education
High School Diploma or GED (Required)
Bachelor's Degree Health Information Management (Preferred)
Minimum Work Experience
2 years Health care experience in medical records of related field (patient statistics). (Required)
Required Skills/Knowledge
Must be PC proficient, particularly in compilation of spreadsheets and reports, with experience in Windows/Microsoft Office Suite, Excel and Access.
Demonstrates excellent written and oral communication skills.
Ability to problem solve and perform in a process-driven environment, with strong organizational skills and an attention to detail.
Must be familiar with the process of medical record number generation.
Must maintain confidentiality with sensitive information.
Demonstrates collaborative and respectful behavior.
Receptive to others’ ideas and opinions.

Functional Accountabilities
Customer Service
- Anticipate and respond to the needs of others; follow-up until needs are met.
- Successfully complete the CNMC Customer Service Training Program.
- Partner with all team members to achieve goals.
- Primary contact for the receipt of identified potential duplicate medical record numbers.
- Investigate and reconcile all patient information relative to the merging of existing medical records in various medical record systems; communicate changes to Health Specialists if necessary .
- Inform all designated parties of the positive identification of a duplicate medical record number and its resolution.
- Contact parents, patient, and appropriate agencies in an effort to confirm patient identity and resolve duplicate medical record numbers.
- Compile weekly reports on duplicate medical record activity.
- Process the legal name change form and update the MPI to reflect the necessary changes.
- Participate in the training and education of name changes and the prevention of duplicate medical record numbers.
- Contribute to policy and procedure development that actively supports the hospital’s performance improvement initiative to minimize the generation of duplication medical record numbers.
